When editing an article that uses Template:Nihongo, the pipe between the kanji and romaji parameters (second and third unnamed parameters) is hidden in the editor. See, for example, https://en.wikipedia.org/wiki/Gen_Urobuchi

I've incorrectly added a pipe character there, which saves as a double-pipe, resulting in a different output than expected. I fixed my error in the edit history.
