There was a recent Swift outage ( T71312 ) during which file move attempts resulted in the page being moved but the file staying at the old name. Those files need to be identified and renamed (or deleted, since some have been reuploaded since). Aaron did some related work (https://gerrit.wikimedia.org/r/#/c/152066/ ).
