From the logs it appears the disk is broken. We need to change the disk and reimage it.

Taking this to replace the disk

The faulty disk has been replaced and a freshly installed with puppet certs and salt. I did not revert the dsh group changes. I will leave that to @Joe

mw1061 is still de-pooled with a link to this ticket, though? Or is there anything else needed to bring it back?

Nope, if it's still depooled it's because I forgot to repool it, I guess.
