References with "equal" entities in their name aren't considered equal when numbering them

Description

If a <ref> has a name that contains an entity, and another <ref> uses the same name but with the entity changed to a normal char, VisualEditor considers them separate, unlike the Cite PHP extension and Parsoid.

For an example, go to https://en.wikipedia.org/wiki/User:Marcoil/Tests/Cite and look at ref number [4], there's two of them, one named "test &amp; me" and the other "test & me". Start VE, they'll be numbered [4] and [5] and considered different in the <references/> list.
