Clicking on 'Cite' in the editing toolbar, then selecting one of the options in the 'Template' menu, seems to be returning a blank pop-up rather than the usual form: see the attached screenshot. I'm finding this on both Firefox 40.0.2 and Safari 8.0.8, on Mac OS 10.10.5.
Checking in the Firefox web development tools, the following error message is being given:
