Make CX translation lists and suggestions language-independent (tied to the Wikidata Q-item)

Description

Recommendations for translation are currently built around the article title in the source language - for example:
https://en.wikipedia.org/wiki/Special:ContentTranslation?from=en&to=ca&campaign=article-recommender-1&page=Mona_Lisa

Is it possible to have the system based around the Wikidata "Q" number for the chosen topic? This allows for greater flexibility, for example, if you know the target language but there are several potential source languages that the end-user can chose to translate from.
So, not " 'Mona Lisa' on en.wp " but instead "Q12418".

I feel that this would make the system more robust, and be relevant to many potential use-cases (though I can't think of them specifically!)

This seems related to T105130: Make finding articles to translate be more flexible (accepting URL and more).

The current task is more focused on using Q-IDs as link parameters, while T105130 is more focused on the users input (and not only for Q-IDs), but when solving one of them it may be useful to keep the related usecase in mind.

This might be dependent on T112769 - "Make it possible to query whether a Wikidata item exists on a particular Wikipedia language edition".
