@matthiasmullie created the extension homepage on mw.org, @Reedy committed the initial code.

I suspect neither of those are still maintaining it?
I also see its used in production. Do we know where?

Would it make sense for us to build this thing in it:

I'm still not 100% sure what this extension does...

It's used on wikivoyage. E.g. https://en.wikivoyage.org/wiki/Rome
Roland Unger & Hans Musil seem to have developed it. I've done some maintenance when setting up Wikivoyage in order for it to work on our setup.
I don't think anyone actively maintains it.

In the left sidebar is "Related pages" with a link to the "Three days in Rome" article. That info comes from the {[related}} parser function, which this extension adds:

{{related|Three_days_in_Rome}}

That's all it does, as far as I remember,

Jdlrobson claimed this task.

Okay, I'm going to advise we build it in this extension and make this extension more awesome. Have communicated my intent on https://www.mediawiki.org/wiki/Extension_talk:RelatedArticles and https://en.wikivoyage.org/w/index.php?title=Wikivoyage%3ATravellers%27_pub&type=revision&diff=2863484&oldid=2863481
