There is a certain pattern in which articles' lead imgs will display the issue :

  • It seems that articles with {{US state}} template display this issue. I checked Kansas, California, Alabama. Generally, articles with infoboxes are prone to display the issue e.g. many articles about college sports{{Infobox NCAA football school}} -
  • but there are many exceptions
    • articles about universities with {{about|The University of Alabama, located in Tuscaloosa}}
    • articles about plants, e.g. Lilac with {{Taxobox }} type of infobox
