MW feedback tool fails to post feedback when you click "File"

Description

The "submit feedback" button opens a form to fill out (v. satisfying). Once you fill it out however, the button at the bottom, "file a technical task", does not use the contents of the form.

It discards & closes the form & opens a "create new task" tab in Phabricator. This may be more confusing if you aren't logged in / don't have a phab account. I was logged into phab, but it was still disorienting and wasted time since I couldn't recapture what I'd written in the form. I thought perhaps it had automatically also filed a wiki comment, until I went to check (open a new form, click link, scroll down).

The "Submit" and "File technical task" buttons should be clearly distinct options, perhaps next to one another, and both should use the form data. New technical tasks should perhaps also generate a brief record on the wiki feedback page.
