I first spotted this locally and thought that's a problem of my local setup. After I haven't found any local problem I tried beta labs with the following URL:
http://en.wikipedia.beta.wmflabs.org/wiki/Special:PasswordReset?debug=true

and got:

This doesn't happen on en.wikipedia.org. It's very hard to debug anything OOUI-related with that :/
