I believe the recommended way is the relevant package in your distro; unclear what shared webserver users can do and what happens if you don't do anything.

I'm not sure about shared webserver users, how do they normally install php extensions that MW requires?
Shared hosting users will have to lobby their host to install the PECL extension (not much we can do about that). Installation notes are on https://www.mediawiki.org/wiki/Manual:Profiling/Xhprof which describes how to enable the profiler.
