Standardize varnish applayer backend definitions

In order to move forward with simplifying and refactoring our configuration, we'd like to standardize on some general rules about application-layer backends:

  1. They're always defined as a singular hostname (not an array, and not a raw IP address). For cases where there are multiple backends, an internal LVS service should be created to handle that, and Varnish pointed at LVS.
  2. They always use the director type 'random'. Others are not useful with a single hostname.

Currently there are only a handful of exceptions, to be linked as blockers here...
