See T118874#2899643 and T118874#2899649. Somehow the task was closed as invalid twice, and the same assignee removed twice. I also received two emails for those actions.

To clarify, I did it using the menu above the comment field, having chosen multiple actions to do in one go.
This is more than likely a upstream issue.
Hmm, It shouldn't let you select from the drop down multiple times, What browser are you using?
You misunderstand, I chose multiple different actions and clicked submit (I pointed this out to let know what exactly I had done).
Mid-air collision - both actions show the same time: "Fri, Dec 23, 21:35".

This is not a duplicate - it's the same user making the exact same change. There was nothing to overwrite.
This has almost certainly been resolved upstream by changes connected to https://secure.phabricator.com/T13054 and https://secure.phabricator.com/D20462.
Closing as per last comment as a more recent software version is now deployed on phabricator.wikimedia.org.
