After upgrade MW to 1.28. (Echo to 1.28 snapshot too, otherwise it generate error message) All the user name showed in page modify notification email become 127.0.0.1.
The email notification work fine before we upgrade MW.
Now it show message like:
Sitename page xxx been modified by 127.0.0.1
in mail title.
What are the exact version numbers of Echo and MediaWiki?

Using Echo-REL1_28-f55bdd9.tar.gz from https://www.mediawiki.org/wiki/Special:ExtensionDistributor?extdistname=Echo&extdistversion=REL1_28
on-wiki notification show correct username, and I got correct name after temp fix for the T156882 . They might not be related. I can't reproduce this error anymore, so set this report to resolved.
