There are a lot of false positives at least in the catalan version (https://tools.wmflabs.org/wikidata-todo/wp_no_image/cawiki.html). It seems small-displayed images like https://ca.wikipedia.org/wiki/Nova_Creu_Alta are not taken into account. Could somebody fix that? Thanks in advance

I wrote the tool. It uses the "page image" information from the respective Wikipedia. For your example, there is no such entry. Ergo, this needs to be fixed in MediaWiki core, or the respective extension.
This is intentional and how it works.
https://www.mediawiki.org/wiki/Extension:PageImages#Image_choice explains how an image is chosen. The image in the lead section of https://ca.wikipedia.org/wiki/Nova_Creu_Alta is not suitable for the type of display the usage of this is intended for.

@ Paucabot No. The alternative would be to use all images on a page, but that would remove too many pages because of navbar icons and irrelevant images, which cannot be filtered out automatically.
