LQT tables might have incorrect encoding

Description

adds /*$wgDBTableOptions*/ to lqt.sql, and updates the readme file to recommend installing the tables through sql.php

The three tables added by LiquidThreads don't respect the table options set in $wgDBTableOptions; this will result in a database error if you have chosen "MySQL 4.1/5.0 UTF-8" when installing MediaWiki. The attached patch adds /*$wgDBTableOptions*/ to lqt.sql, and updates the readme file to recommend installing the tables through sql.php.
