In my browser (Firefox on Linux), 3rd-level (===) headings look just like 4th-level headings (====).
E.g. the headings under "Prose" here:
https://en.wikipedia.org/wiki/Boris_Vian?useskin=timeless#Selected_bibliography

One of the recent forms patches where I messed up the fonts should have fixed this (probably https://gerrit.wikimedia.org/r/c/mediawiki/skins/Timeless/+/504963). h4-6 are all fairly similar now, but honestly that's not unusual...
