Create a functional-looking mockup for Special:Block datetime selector

Description

We want to demonstrate to users the changes we want to make to Special:Block in T132220 in a testable environment that does not require permissions.

Acceptance criteria
  • We will need this demo to function for ~4 weeks. This can be a throwaway HTML page that expires after a few months, whatever is easiest to build.
  • The form should match the Special:Block interface and tab order, with the proposed changes in T132220
  • No data validation is required for username, reason, etc.
  • The blocks should not actually be set — this should not be hooked up to any wiki. This is just for UI usability testing.
