I think that not only the height is too small, but also the width. If ilo is lengthier than en (which probably is the case), the toolbar will wrap into two lines, the insert menu being in the second, thus decreasing the available height for the menu by one item. (Modify the width of your browser window while the menu is open, and you'll see what I mean.) This also explains the fact why the ilo screenshot has no help and page notice icons.
