https://meta.wikimedia.org/wiki/Special:CentralAuth/DeannaPeter3 was created only locally on enwikiversity without any accounts. Why did this happen? If there's a method of reproducing this behaviour, this will gravely disrupt our ability to address spambots as they would not have any log on loginwiki for us to monitor the spambot issue globally.

I just discovered a very similar case when locking [AureliaSperry](https://meta.wikimedia.org/wiki/Special:CentralAuth/AureliaSperry). Nothing of interest in the logs at a glance (no obviously related exceptions, fatals, …).
Interestingly also no CentralAuthCreateLocalAccountJob Job seems to have been schedule: hoo@mwlog1001:/srv/mw-log$ grep CentralAuthCreateLocalAccountJob JobExecutor.log | grep AureliaSperry yields no results.
