Steps to Reproduce:
Add a note on a file in Commons
Actual Results:
Exception Error: Version inconsistency after saving. Please reload the page.
Could not save your note (edit conflict or other problem). Please copy the text in the edit box below and insert it manually by editing this page.
I'm able to save the note with a page reload, however.

I'd have no idea how to add a note (and that this requires enabling some gadget) if I had not come across this issue a few days ago in T226500. Thanks.
