Given the current activity in T&S project I think we need a dedicated project for password, email and 2FA reset requests. "2FA-Reset" may be added as an alias.

I'm not really sure how this would help other than by adding a more intuitive name for people to look for in the projects box when filing a task.
This can be created as a milestone of Trust-and-Safety project so it is both displayed as an column of parent project and a board of itself with columns.
I'm sorry, but this is not likely to be picked up by T&S. We have an annual plan's worth of projects to fulfil as it is. We can discuss it but I cannot promise anything will be put forward.

I'm proposing the project in requesters' perspective only. They should find the correct project easily. A form may be preferred so that it takes no efforts to categorize these things once there's adequate instruction.
The requester's perspective might require better on-wiki documentation to find out where to request things (e.g. in something called "Phabricator").
I'm not sure where any such password reset workflow (and data requirements) are currently documented on-wiki, looking at pages like https://en.wikipedia.org/wiki/Help:Logging_in#What_if_I_forget_the_password? or https://wikitech.wikimedia.org/wiki/Password_reset (I am failing to find a good page on metawiki), plus misleading off-topic pages like https://www.mediawiki.org/wiki/Password_reset or https://meta.wikimedia.org/wiki/Security/Password_reset
Adding more administrative burden on others by creating random Phabricator workboard columns that nobody plans to use does not solve any problem at all.
I think you are totally correct that it can be very hard to find out where to request a password reset (and maybe which data is required).
But that is a problem to solve on wiki, so people manage to find Phabricator.
On wiki, you can use [[phab:maniphest/task/edit/form/1/?projects=Trust-and-Safety]] to create such a link.
It is not a problem to solve in Phabricator if people do not find Phabricator anyway.
So I'm going to boldly decline this task.
