Scale: create a Phabricator template for deployments

Description

Scaling led to a simplification of the deployment process. But this process still needs a Phabricator task for each deployment.

This task to create a Phabricator template communities can create, fill and edit if needed. Should be based on the previous Phab template we used.

@Trizek-WMF -- do you think we will still need such a Phabricator template once we do T246939: [Growth scaling] streamlined configuration? If we do that work right, then communities can fill out an on-wiki configuration, instead of a Phab task, right? Or will we still need a Phab task?

My goal is to have a way to create a task similar to T252420: Scale: enable Growth team features on French Wikipedia only using a couple of clicks.

It would be a good way for communities to document if a change has to be made concerning help pages or templates. We can go to the community and tell them "please go to this Phab task before the deployment to change the configuration".

(There are either templates with many funny URL parameters, or see and follow https://www.mediawiki.org/wiki/Phabricator/Forms instead.)

I re-created a template for this task. Since it is based on T252420 and Catrope will use this task, he will also crash test it.
