This would create a boolean option in Special:Preferences:
"Prohibit site blocked users from emailing me" (off by default, may be a global setting with local overrides)

I dropped a note on the request. I am wary of adding more preferences, honestly. I have noticed that far too many preferences just get lost there with nobody ever seeing them.
I'm not sure this is really a good idea, but filed it for the other person. Users that are misusing email can be blocked from email, and if they are only bothering one person they can be manually muted (not to mention that in WMF wiki's if you don't use this option globally they can just email you from another project - even one where they are not blocked).
I wonder if this should be an option of the block rather than a user preference. Arguably if you are sitewide blocked, you shouldn't be able to email anyone (other than I guess people who have the block right)
@dbarratt there is already an admin option on blocks, to remove all sendmail access from the blocked user, this would be for an individual that doesn't want email from siteblocked users even if that option isn't selected.
