This doesn't look like a failed disk but a BBU failure.

Indeed! Looks like BBU failure to me too, @Papaul please replace; host can be powered off at any time

@fgiunchedi BBU replaced, server is happy now

reopening as I noticed the battery status is still recharging, though I think that's unusual and surely should have been charged by now?

Thanks folks for merging the duplicate tasks. I've temporarily disabled the event handler on icinga so no further tasks will be open.

@Papaul this BBU is flipping between recharging and OK apparently, do you have other BBUs or we could buy one? thank you
