Create a form for tracking administrative privilege requests for GitLab

Description

This has come up a few times for GitLab.

I'm loathe to add more bureaucracy around here, but it might be reasonable to track this sort of thing in Phab.

Also possible that something already exists - filing this ticket as a placeholder for a bit of future research.

For Gerrit requests are filed against Gerrit-Privilege-Requests or for MediaWiki repo against MediaWiki-Gerrit-Group-Requests . The policy is at https://www.mediawiki.org/wiki/Gerrit/Privilege_policy

@brennen: What is wanted or left in this task? Forms 105 (GitLab project group request) and 117 (GitLab Account Activation Request) exist.
