The option

$wgUploadWizardConfig['blacklistIssuesPage'] = 'Commons:Uploadwizard_blacklist_issues';

should be set as part of the UploadWizard configuration block in CommonSettings.php. This will allow users to report false positives resulting from the title blacklist.
