Created https://wikitech.wikimedia.org/wiki/Template:Account_setup and made revisions to all the aforementioned pages. The Toolforge account creation process is still convoluted, so the instructions at Toolforge/Quickstart are not great. Changing the process is on the agenda for the next Toolforge workgroup meeting, and is out of scope for this docs task (though if the process changes the doc should be updated to be simpler and clearer).
