You write to the existing channels (VRTS, steward noticeboard; maybe UTRS could also be reused) with your IP and what username you want, someone checks the IP info, decides you are OK, creates the account for you and makes it ipblock-exempt.
For that, the message should probably include the IP address. It's useful both for admins to know why the IP got denied (I imagine using a proxy and someone having done web scraping from the same IP would be handled differently), and so they can better report problematic blocks to us
