I am noticing some inconsistencies around the state of Insert button in various VE dialogs.

For some dialogs, the Insert button appears enabled even when there is nothing entered in the dialog. These are: Comment and Code block.
They should stay disabled when you open these dialogs through Insert action.

For some dialogs that have default values already present on opening, should have the Insert button enabled. These are: Map and Graph. Right now the Insert button for Graph stays disabled when you open this dialog through Insert action, even though it has all the default values populated that are valid enough to add it.
