Description

Implement the new design for the IRS emergency flow as detailed in T360986.

Acceptance Criteria

A new screen allows the user to enter the emergency flow by selecting between two radio buttons
Three incident sub-categories are available under a dropdown associated with a radio button
Incident report is sent to emergency@ for review
Appropriate Codex components are used where available
