Description

Some bugs reported will refer to the page the user was viewing
e.g. the table is rendered terribly

It would be good to have more context
Other information that would be useful is knowledge of whether javascript/images are disabled.

Wikipedia username if available should also be added (possibly allowing a user to input it manually if they are not logged in)
Revision number might also be useful for feedback about errors in articles (especially where spam is concerned to have happened)
