Tweak design of Simple summaries on desktop

Description

The initial design for simple summaries had a "Was this helpful? Yes/No" affordance below the disclaimer copy, but this is similar to a common help article knowledge base pattern. There's a risk that readers think the "Yes/No" prompt is referring to the disclaimer, rather than to the summary text. I want to tweak the design to fix the gestalt of the summary window so that the "Yes/No" prompt refers more clearly to the summary text, and not the disclaimer. This involves adding a key line and moving the Yes/No buttons up above the disclaimer.
