Description

We would like to run a survey for users viewing the moderator dashboard. This should probably always be available for users to answer (perhaps after meeting some usage/activity criteria, see 'Open questions' section), so we don't need to use QuickSurveys here. It could just be available in a banner or popup.

We need to both design where this survey will live on the dashboard, and create its content.

Open questions

Should users need to meet some criteria before seeing the survey, like visiting the dashboard more than once?
