We need to create the survey that we will present to users of the moderator dashboard. This can be an externally hosted survey, for example in LimeSurvey, which we link to.

We should try to avoid being too specific in our questions, so that we can continue using this same survey for as long as possible, even as we iterate on the dashboard's features.
