All gnmic instances have been upgraded to 0.45.0

Manually tested on netflow4003 and works well.

Two differences:
The metrics/graph sum by (source) (rate(gnmic_subscribe_number_of_received_subscribe_response_messages_total{site="$site"}[5m])) doesn't work
It has been replaced with sum by (target) (rate(gnmic_targets_subscribe_response_received_count{site="$site"}[5m]))

I've updated the graph to show both at the same time.

sum by (grpc_code) (irate(grpc_client_handled_total{site="$site", job="gnmic"}[$__rate_interval])) is not exposed anymore, so this graph will need to be removed, but that's not a big deal.
