Steps to replicate the issue (include links if applicable):

• Add multiple usernames to Special:MultiLock
• Uncheck at least one of the checkboxes in the table
• Submit the form, but have it fail for some reason (like a loss of session data error)

What happens?:
The rest of the form is the same and can be immediately re-submitted, but the checkboxes reset themselves to being checked again.

What should have happened instead?:
Un-checked checkboxes should remain un-checked on error.
