UploadWizard does not set filename for stash upload AJAX request

Description

Sometimes when a file is uploaded via UploadWizard, the AJAX request to the stash upload API has not filename set. This results in bad filename errors in various circumstances.

Seems to be intermittent, or maybe browser version specific.

This is an old-ish bug that I'm not convinced we have enough information to replicate - examples? Steps to reproduce? Which browsers it's specific to?

If @Tgr can enlighten us then I'll raise the priority but right now it's no good.

I don't remember what this was about, but seems like it was fixed in the subtasks.
