Note that in the legacy forms, we've added an HTML comment which reports the relative file path of a form partial each time one is transcluded. We need something like this for the Mustache forms.
We need to have syslog record the same information.

Given the small number of partials, and the departure from core templating that would be necessary to automatically add these, let's just put an HTML comment with filename at the top of each template.
