I don't think a similar problem has occurred before. I noticed it for the first time in the case of Sławobóg (second screenshot)

I was following the lists on https://meta.wikimedia.org/wiki/Oversight_policy#Requests_for_oversight where they are in format: oversight-lang-project. But then I think wikipedia-pl-oversight will be ok.
