The only pages with "create page" form links have the information in them to fill in the basic information for the pages being created. I mostly was emulating the multiple page edit on a page with multiple-instance template fields, because the wiki I am making will not have many computer savvy people editing it so I was trying to keep actual input as simple as possible. At this point I've just set the forms to not be auto-created and people have to click through and hit the submit button instead of it just being made automatically. It's an extra step, but this way is needed to prevent the huge pile of spelling errors that would propagate and cause issues with SMW.
I had thought that default values were static from the form page, not something that would allow the link to have different values for each page being created..

I'm not sure if this helps but I actually hacked mine to work with autocompleting from a list of values using information on this page: https://bossanova.uk/jexcel/v4/examples/dropdown-and-autocomplete
