After testing the introduction of reduced address data provision, we want to test the effect of asking a user which address data they want to provide in the banner form already. Banners are based on the **control|variant banner** of **desktop-de-nn**.
**Acceptance Criteria**
* The variant banner has a two-step form (similar to the variant of desktop-de-02).
* If a user selects direct-debit,
* only the full address option is available, other options are disabled.
* the form shows a notice below the headline that explains why (direct debit notice).
* The submit button label of the second page changes depending on the user's choice.
