The link on http://tools.wmflabs.org/ to the form to request a tool labs user is broken (see parent task).
Meanwhile, let's move the user registration over to phabricator whether we keep using SMW or not.
- create a process (project tags) in phabricator that replaces https://wikitech.wikimedia.org/wiki/Special:FormEdit/Tools_Access_Request
- change the broken link on http://tools.wmflabs.org/ to point over here
