Could someone give more context? Is this a particular issue with specific countries? Can I get a fictional example that is not a donor?

Basically it's just typical ways people shorten their names in places where the Spanish naming customs are the norm. Or at least in Mexico... though I just saw that there are differences in other parts of Latin America. So, fictional example, someone whose full name is José Antonio Hernández Morelos (two first names and two last names) might often shorten it as Antonio Hernández. Hope this is helpful!
