collapsed tables should display show/hide in caption not first cell

Description:
Putting the JavaScript show/hide button in the first <thead> cell of a table often can make the table look messy.

There must be a better place to put it - indeed I think there is:

If the table has a Caption (marked within <caption></caption>) let's put the [show/hide] button at the end of that caption.

At the moment, I suspect it'd probably be easiest to continue to display the top row of cells - though making them disappear as well is something I've thought of (I think that's probably a 2nd stage).

For example:

My shopping list [show/hide]

! bla bla bla etc
