TOC numbering can be wrong

Description:
Using the wrong heading order (say a "===" before a "==") confuses the parser,
and numbers in the TOC can be wrong. See [[:it:Utente:Alfio/Sandbox]] for an
example. The TOC in that case is:

1 title 1
2 title 2
1 title 3
1.1 title 4

Incorrect headings will produce incorrect TOCs because they are incorrect. The
solution is to not use incorrect headings.
