Steps to replicate the issue (include links if applicable):

• Disable JavaScript
• Go to https://commons.wikimedia.org/wiki/Special:UploadWizard

What happens?:
The Patent permissions form appears in the fallback to Special:Upload

What should have happened instead?:
Special:Upload does not display the patent permissions form (rETHR modules/ext.3d.special.upload.less), so the fallback to it on Special:UploadWizard shouldn't either.

It appears to be causing confusion: https://commons.wikimedia.org/wiki/User_talk:AntiCompositeBot#This_bot_marks_pages_that_are_uploaded_with_released_rights_as_having_insufficient_copyright_information.
