Number of endnotes does not match number of references if duplicate references

If more than one reference has the same content, only one end-note is created.
This can result in a mismatch in number of references and endnotes.

Please provide a *simple* testcase (hint: something that isn't ~30KB) and
specify how the output that's being generated differs from what you think it
should generate.
