Create a Campaign for Program Dashboard

Create a Campaign modal/page for Programs Dashboard. This is essentially the same as the current Create a New Program modal. This page includes some reassurance that you'll be able to add other people as co-campaign organizers. The information entered here will populate the fields on the Campaign:Overview page in T148804. This form does not include a space to add the Program Template, because it's too complicated to explain and too stressful to produce on this form -- that will have its own call to action on the Campaign:Overview page. {F4705362}
