I updated the Survey Goal section to remove references to quarterly, engineering-wide surveys.

I added new content to describe the "Rethink" under the Implementation Details section.

@ksmith @JAufrecht are either of you available to review the changes I made to https://www.mediawiki.org/wiki/Team_Practices_Group/Health_check_survey ?

Looks good to me. The FY1516 notation is deprecated, though.
